Taking Control
At Hayes Green Beach Memorial Hospital, we want to improve the quality of life for patients with diabetes by providing an educational program within Eaton County to guide self-management, thus preventing and limiting long-term health complications.

Newly diagnosed individuals are taught vital tactics:

  • Medication administration
  • Diet guidelines
  • Blood-sugar testing

The objective for the individual is to be able to manage safely at home until he/she can be enrolled in the classes. Call scheduling at 517-543-1050, ext. 1618, to arrange an appointment.

Individual Services
Patients can schedule appointments with the nurse and dietitian to discuss individual self-care and meal planning. Appointments can be made by calling scheduling at 517-543-1050, ext. 1618.

Diabetes Education Class Series
The curriculum of this class is designed to promote appropriate self-care based upon state and national standards. Classes are taught by registered professionals, including a nurse, dietitian, pharmacist, and physical therapist. Topics include the following:

  • Nutrition and meal planning
  • Self-monitoring/glucose testing
  • Personal health habits
  • Complications – acute and long-term
  • Physical activity
  • Reduction of risk factors
  • Medications
  • Learning to live with diabetes

Diabetes Support Group
This free support group offers assistance to all individuals with diabetes in the community. It
is a great time to meet and share with others and learn from knowledgeable speakers. Presenters vary each month. Dietitians, nurses, physical therapists, and pharmacists discuss topics ranging from new medications and treatments to healthy holiday eating.
