Whether you're recovering from a fall, managing a chronic illness, or in need of help to administer medication, HGB's Homecare Program can be an essential link to getting you up and going again.

Variety of Skilled Services
The accredited Homecare Program at Hayes Green Beach provides a variety of skilled services to help you or your loved one reach the maximum level of recovery from an illness or surgery.

Qualifying for Homecare
To qualify for care at home, a patient must:

  • Have a physician’s order;
  • Be homebound;
  • Have a need for skilled care; and
  • Require services for a limited time period.

The Mission of HGB Homecare
Our mission is to provide quality care to our patients throughout their lifespan, in their homes, while emphasizing the maintenance of health and wellness.

Services Offered

  • Skilled nurses, who perform tasks such as dressing changes, IV therapy, and medication monitoring, and who will teach you and your caregiver how to continue with your care
  • Physical therapists, who help you become stronger and better able to remain independent
  • Occupational therapists, who may assist you to adapt your environment to allow independence in caring for yourself
  • Speech therapists, who can help you with problems related to swallowing and speech
  • Medical social workers, who link you with community resources that will help you remain in your home and assist with long-range planning
  • Home health aides, who can assist you with personal care and exercises established by the therapists until you are stronger and can perform these functions independently

Access to HGB Homecare
HGB Homecare is easily accessed through a referral by your physician. Nurses are available seven days a week, 24 hours a day.

An assessment visit will be performed by a registered nurse and a plan of treatment will be established with input from your physician, your caregiver, and you.

Our goal is to return you to your optimal level of health.
