New Membership
Health Risk Personal Assessment
If you are interested in joining the Wellness Center it is important for us to understand your current health status. Once this form is completed we will be able to begin processing your membership.
Medical Clearance Form
If after completing your Health Risk Personal Assessment Form you find that medical clearance is needed you may present this form to your physician for his or her signature.
