At Hayes Green Beach, we want to improve the quality of life for patients with diabetes by providing an educational program to guide self-management, thus preventing and limiting long-term health complications.
Newly diagnosed individuals are taught vital tactics:
- Medication administration
- Diet guidelines
- Blood-sugar testing
The objective for the individual is to be able to manage safely at home until he/she can be enrolled in the classes.
Patients can schedule appointments with the nurse and dietitian to discuss individual self-care and meal planning.
Diabetes Education Class
The curriculum of this class is designed to promote appropriate self-care based upon state and national standards. Classes are taught by registered professionals, including a nurse, dietitian, pharmacist, and physical therapist, and meet each Monday with rotating day and evening classes. The classes are broken up into four parts with topics including:
- Nutrition and meal planning
- Self-monitoring/glucose testing
- Personal health habits
- Complications – acute and long-term
- Physical activity
- Reduction of risk factors
- Learning to live with diabetes
Diabetes Support Group
This free support group offers assistance to all individuals with diabetes and their families. Healthcare professionals and speakers present topics ranging from new medications and treatments to healthy eating.
The support group meets the fourth Thursday of each month from 6:30 p.m. to 8 p.m., at AL!VE.
